Services

I provide person-centered planning, support for entering California's Self-Determination Program (SDP), and Independent Facilitation after enrollment.

The participant directs the work.

Person-centered thinking is the foundation

Person-centered thinking starts with the participant—not a list of services, forms, or budget categories.

Person-centered planning

Person-centered planning can help make visible:

  • how the participant communicates and what makes access work;
  • strengths, routines, relationships, and community;
  • what matters to and for the participant;
  • what is working and what is not;
  • what the participant wants to change, preserve, or make possible; and
  • what support may help move toward those outcomes.

A written Person-Centered Plan can capture that work when it is useful or wanted. The planning process and the written document are not the same thing.

Transition into Self-Determination

Transition can involve several things moving at the same time.

Depending on the agreed work, I may help organize:

  • person-centered planning and IPP preparation;
  • the individual budget and Spending Plan;
  • FMS selection and setup;
  • providers or workers;
  • rates, hours, qualifications, and implementation questions;
  • records and timelines;
  • meetings and follow-up; and
  • what still has to happen before services can begin.

Transition should lead to usable supports, not just completed paperwork.

Independent Facilitation after enrollment

The Independent Facilitator is selected and directed by the participant.

Depending on the work we agree to, I may help with:

  • informed choices;
  • person-centered planning and IPP preparation;
  • individual-budget and Spending Plan questions;
  • locating, accessing, and coordinating services and supports;
  • identifying immediate and longer-term needs;
  • developing options;
  • FMS, providers, and workers;
  • meetings, records, and participant-directed communication;
  • advocacy within the Independent Facilitator role;
  • follow-up on decisions and authorizations; and
  • implementation when approved support has not reached real life.

The participant directs the priorities.

When something is stuck

I may help answer four questions:

What was decided?
What happened afterward?
What's missing?
Who needs to act next?

Access and communication

Communication and access are part of the work, not an add-on.

Access & Communication

Current rate

Post-enrollment Independent Facilitation: $100 per hour

Pre-enrollment services use the funding arrangement that applies to that work. Scope, rate, and billing are confirmed before substantive work begins.
